"Surgery" literally means to work with one's hands; thus in the medical sense it implies the treatment of disease using the hands. With the exceptions of the skin and mucous membranes, the internal organs can only be accessed by opening up the body - therefore surgery became synonymous with cutting or opening up a part of the body.
